Project Healthy GrandparentsIf you do not reside in south Fulton or DeKalb County, Georgia, go to the “More Resources” section of this website and click on AARP: Grandparent Information Center for services in your area.

If you reside in south Fulton or DeKalb County, Georgia, complete this form to make a referral or to request to participate. If you are a grandparent, fill out the sections down to “Referral Sources”. All others should complete the entire form. Click on "Submit" when finished. 

Eligibility Requirements
To participate in this project, grandparents must be:

  1. Residing in South Fulton or Dekalb County, Georgia
  2. Raising one or more grandchildren 16 years of age or younger
  3. Living in a home where both parents are absent.
