Both versions must be printed and mailed in to the Honors & Awards Committee.  The PDF format requires Adobe Acrobat Reader to open.  The .DOC format will open in Word or Internet Explorer and can be edited on-screen and then printed to mail.  All nominations are due by December 20 and all awards will be presented at the Annual Meeting in January each year.
