Both versions must be printed and mailed in to the Honors & Awards Committee.  The PDF format requires Adobe Acrobat Reader to open and must be printed then filled in offline.  The .DOC format will open in Word or Internet Explorer and can be edited on-screen and then printed to mail.  All nominations are due by December 15 and awards will be made at the Annual Meeting in January each year.
