Forms may be completed online, printed and submitted to the Benefits office by November 2, 2007. Please note that
original signatures are required for all enrollment forms.
To see if your provider participates with the BOR HDHP, Go to: www.bcbsga.com, select the BlueChoice Preferred Provider Organization (PPO) in GA, or if looking outside of GA, select the National BlueCard (r) Doctor & Hospital Finder.
