Syllabus Statement
It is the policy of Georgia State University to adhere to the letter
and spirit of the Americans with Disabilities Act of 1990 and Section
504 of the 1973 Rehabilitation Act. To assure such compliance, the
Office of Disability Services (ODS) was instituted. However, it
is everyone's responsibility to assure that all students know that
GSU has an Office of Disability Services and where the office is
located.
The following statement should be included on all syllabi or distributed
to all students along with the syllabus. It should be read aloud
by the faculty member to the students during the first week of class.

Accommodations for students with disabilities:
Georgia State University complies with Section 504 of the
Rehabilitation Act and the Americans with Disabilities Act.
Students with disabilities who seek academic accommodations
must first take appropriate documentation to the Office of
Disability Services locate in Suite 230 of the New Student
Center.

It is the responsibility of ODS to assess the documentation of
each student requesting academic accommodations based on disability.
When the documentation substantiates a disability, ODS decides
which academic accommodations "level the playing field"
for that student. The student will then be given an "Accommodation
Request Form" (Note: this link will open up in a separate
browser window) stating the nature of the approved accommodations.
Any student, requesting academic accommodations from an instructor,
should present their "Accommodation
Request Form" (Note: this link will open up in a separate
browser window). Until you receive this form, you are under
no obligation to provide accommodations to the student.
When you receive the "Accommodation
Request Form" (Note: this link will open up in a separate
browser window), you can be assure that the student has presented
ODS with proper documentation and that accommodations listed are
appropriate for the individual student. If you have any questions,
please call Disability services and ask to speak to the staff person
who signed the form or the Director.
Testing Accommodations:
When testing accommodations are indicated (i.e extended test time,
testing in a distraction-reduced environment, etc.), you will be
asked to fill out a "Request
for Individualized Testing Accommodations Form (RITA)"

It is the student's responsibility to bring the RITA to ODS at
least 48 hours before the first scheduled exam date.
As a courtesy, testing accommodations are often handled through
the Office of Disability Services. We can also make arrangements
to pick up the exams from you or the department and then deliver
back when finished. Faculty indicate their preference for such arrangements
on the RITA Form. Unless otherwise indicated, it will be assumed
that the instructor will deliver and pick-up his/her exams from
ODS. There is also an area to indicate what a student may
take into the exam room with them.
