Nomination Information Sheet

Employee Development and Wellness Services is currently accepting nominations for its annual Reward and Recognition Program.

Fall 2008 Deadline Extended: October 01, 2008

PURPOSE: To recognize and encourage those behaviors that lead directly to the accomplishments of the division’s stated goals.

Select appropriate catagory: (NOTE: if nominating in more than one catagory, please submit as many times as needed and indicate specific catagory. You can also Download the Nomination Sheet in PDF format, fill it out and Email or fax it to our office)

The Bright Idea Award: This award is designed to recognize those individuals who e an innovative idea for a new initiative or solution to a workplace challenge, big or small.

The Process Improvement Award: This award is designed for individuals or groups of individuals who have brought about improvement of a unit, division and/or university process.

The Exceptional Customer Service Award: To recognize individuals who have demonstrated exceptional service by going “above and beyod” one’s job scope.

The Professional Improvement Recognition Award: Honors those employees who have taken the initiative to better themselves through education.
